You have the right of access in order to inspect and obtain a copy your personal health information contained%3cbr%3ein your designated record set%2c except for %28a%29 psychotherapy notes%2c %28b%29 information compiled in reasonable anticipation of%2c%3cbr%3eor for use in%2c a civil%2c criminal%2c or administrative action or proceeding%2c and %28c%29 health information maintained by us to the%3cbr%3eextent to which the provision of access to you would be prohibited by law. We may require written requests.
